The top of a fist signifys the fresh and painless act of and or beginning to enter one's fist in an anus. The top of the fist has not yet entered the anus and therefore has not caused any damage and or pain signifying joy and what is good. An antonym would be "Bottom of The Fist"; which would the exact opposite, signifying pain, anguish, and defeat.
