a term to describe an out of touch/delusional/stupid elderly person, that acts weird. "lead paint stare" is something some old people have, making them look like they're not really there, said to be an effect of lead poisoning.
why its used to describe old people is because someone near the age of your grandmother, most likely had their walls painted with paint, which contains lead, hence the lead poisoning.
