4. Out of control. Damn near dangerous.

These nurses at the institution are so bad at their job they let the patients act out and do as they wish, however, they say that their caring but they are complicit in their actions. There is a lack of safety, there is tension and there is apathy. This can only be described as ultra madness.

Neo madness refers to an era after the "death" of the Madness Combat community which consists of people having a surface level interest of Madness and making it like every other fandom. The creator of this attribute the definition to current memes, shipping, and furrifying characters.

This definition has been watered down in multiple ways, the most notable being it attributing it to the spiritual death of madness, despite the named "death" being described before the neo madness era.
