Breaking your clean gives the meaning of getting back into a bad habit. This can include getting back into drugs, self harm, or giving up rehab (mainly self harming again after doing it before after a long time).
“I noticed you were drinking again last night. Did you break your clean?”
-“I did. I might need to go back into rehab to fix it.”
