To have a baby on someone means to have a child with someone other than your partner that you didn’t acknowledge them about. If your partner didn’t know when you did then you had a baby on them because you didn’t tell them.

“He had a baby on me!”
“He had a daughter that’s older than our son that I didn’t know about.”
