Billy can be used as an insult, but more often is used between friends coming from different areas. Often times referring to rural areas.

Some people living in a city may refer to the suburbs as Billy also.

If you’re from New York all of New Jersey is

Billy AF. Non specific not derogatory but individually may be.

ANYONE can be Billy it’s not race or age specific similar to how rural communities use redneck. It’s more of a vibe and has become pretty inclusive depending on where you’re at, but people in the city or even suburbs will generally think you’re meaning a white person if you say redneck.
