Genuinely painful understanding 

An understanding of a concept so strong that you have a disturbing understanding of it, no no no you don't get it like I mean someone can still know about it, but not know as much as you do, usually in a depressing "oh godi get it now" sort of manner, the more of this painful knowledge is amassed the worse ones mind becomes.

gary: oh god i understand.

larry: and that bad because?

gary: everyone thinks that blood relatives are more important

A term used by idiots to make playing with emotions to sound more positive but in the end, they are fooling each other and cry like an elephant when his/her partner found another idiot to play with
