You say 33 year old dwarf in conversation to describe something shocking, unexpected or surprising (something that you did not see coming - see example 1). It could also be used to describe an unexpected development.

Taken from the rather unexpected plot twist in the film 'Orphan'.

Nicaraguan dwarf surprise

I was at Panera, eating lunch with my family. I went to the bathroom and my pesky 3 year-old son tagged along, stood in between my legs, and watched me pee into the urinal. Seizing the opportunity, I completed a Nicaraguan Dwarf Surprise by farting on his head. We both laughed.
