It used to mean that an edit was made to a program from a third party. These hacks were used in video game terms to be synonymous with cheat codes.
Then the normies got a hold of it.
Now it pretty much means a clever trick, although with some of the ways that it is used, the 'trick' usually isn't clever at all.

To hack, to cut through with cleverness, to bend things to your will. Not the bad kind, the alive kind. When you're bored in a meeting, doodling on a napkin, that's hacking time.
