That's sick can have different meanings, so you have to be conscious of the context and the way it's expressed.

1; Can be used to express awe or admiration for something interesting.

2; Can be used to express a disdain for something not-very-pleasant.

The "That's Fricken Sick"also known as #TFS is a hand gesture and saying used only by the sickest people. It is displayed easily, you do a thumbs up and throw it down into basically a Shaka or the "Hang Loose" sign, but with your thumb bent downwards at the knuckle. Accompanied by the phrase"That's Fricken... SICK!"
