Used to define something disgusting and is typically used as an insult, to say someone drinks butt juice. Butt juice has no limits and can be used anyway or be anything you want (but it is typically a liquid).

This is in contrast to more popular types of men’s underwear, such as boxer briefs, which are tight around the legs rather than the butt, and regular boxer shorts, which are not tight at all and therefore do not hug any part of the body.
