Like a scratched CD gets stuck in one place and repeats the same sound over and over, a (metaphorically) scratched person broods on something for hours and hours, wondering and worrying too much about it.
Usually people get scratched about small matters, like that stupid thing they said to their crush and they regret. However, it's possible to get scratched about important things, for example, a final exam that went horrible.
