This term most often occurs in hospitals that trim their patients down by who has insurance, or any history of drug addiction thusly allowing him to only treat those that he finds aesthetically pleasing.

A suicidal patient arrives at a hospital, makes a desperate plee for help so that they do not injure themselves but are slightly intoxicated. The psychiatrist will then have the police take the client into protective custody where they are held in a prison cell while the client with insurance gets the hospital bed.
