Pain in the Bum

This is British slang and is a slightly less vulgar way of saying pain in the arse or pain in the ass if you are American. In this expression the bum in question isn’t an American bum, a word that dates from around the mid nineteenth century, but an English or Australian bum, a word that dates back to the early seventeenth century. At one time the word was not considered to be vulgar at all and was used extensively during the seventeenth an eighteenth centuries. One Gentleman Soldier, fighting for the King during the English Civil War, wrote in a letter that on one occasion the going as so difficult that he “slipped and fell on my bum”.
Putting it politely, Malcolm, you are nothing but a pain in the bum!”

Pain in the bum

Pain in the bum is a British slang equivalent of pain in the ass. Usually the result of a traumatic, painful event, this is the type of pain that hits your deepest insecurities.

On the outside you may seem fine, smiling and acting normal, but inside you're grieving and hurting.
Soul-deep pain may not be reflected on the outside, but can typically be seen in the person's eyes.

They tend to look blank and hollow, despite their seemingly genuine smile.

pain can be mental or physical. sometimes when it’s mental people change it to physical because mental pain hurts a lot worse than any physical pain.

pain is caused by life i guess, human nature.
maybe you fall, or get cut or bleed.
or maybe you feel empty, sad or hopeless.
these are all examples of pain.
