It is the equivalent of calling a Man a sir, only that in this case you are reffering to a intersexual or a hermaphrodite.

Saddam hussein was not a sadam, because he didn't act in the appropriate ways which would define a sadam, but it doesn't mean that he wasn't a hermaphrodite.
