When the accused party "investigates" itself instead of allowing an outside investigation. Generally the results of the investigation are pre-determined by the investigating party to avoid responsibility.
US Army bombs hospital in Kunduz and refuses an independent investigation with Pres. Obama giving his blessing. The inventigation results were leaked 2 days before the official release, the day before 2015 Thanksgiving, and can you guess the results?
