1. Usually what a girl says when asked how she is feeling, and she feels like a sack of shit not even worthy of the toilet, and she is trying to conceal her feelings but she's not very good at it.

2. This is more common, when a girl is obviously not feeling well, she will say she is fine simply because this word lets people know she is definitely not ok. But sometimes people can mistake this for true happiness. (Usually not an accident.)

1. Tristan: Hey Alexa, how are you?

Alexa: fine.

Tristan: Well that's great! What did you do today?

2. Tristan: Hey Alexa how are you doing?

Alexa: fine.

Tristan: No you're not, I can tell something's wrong, what's bothering you?

Alexa: nothing, I'm fine.
