Loaded for Bear 

To be prepared, mentally and/or physically, for extreme opposition; typically used in reference to an aggressive or potentially violent situation.

The phrase originates with American hunters and woodsmen in regions frequented by the brown bear. Brown bears are the largest land-based predator on earth, and when expecting to deal with them the hunters would bring much more powerful rifles than they would if hunting other game.
When the police responded to the reports of a men in body armor with AK-47s robbing a bank, they came loaded for bear.

penny loafer 

is a kind of shoe, like an oxford. Other countries may call it mocassin. They are called penny loafers because in the old days the shoes had a slit on top of it where a coin, a penny, a dime could be fitted, specially if wore by girls, in case they needed to make a phone call.

Lisbon Loafers 

Term adopted by fans of CBS-TV's police-procedural drama, THE MENTALIST. Came from a typically offhanded slighting remark by title character Patrick Jane (Simon Baker). Refers to the kind of just-barely-dressy beachcomber-type sandals often worn by Teresa Lisbon, co-star (Robin Tunney).
