The two things must be equally bad and happen in close relatioin to one another or in sucession. The origin of the story is at an event where a lady shit on the floor and then a dude came up and puked on it.

when two bad things happen to compound one another. Like if you see shit and it makes you puke. When things are already going bad and something comes along and makes it worse. Can be an action of puking on shit. Like kicking someone while they are down.
