Someone is lying to you when they say they're okay. They're not okay. Don't fucking ignore someone if they say they're fine. Talk to them. I know this because I myself say I am fine even though I'm not in anyway.

The origin of this phrase.. well, okay- so, typically on the "Hub" before the storyline has a twist and switches to the juicy part of the video, the main actor in the video says this phrase. The use of this phrase isn't meant as a reference to this, but when said and pronounced correctly by males it can make them sound gay in a humorous, joking way.
