ER is used to describe when a persons actions are so outrageous that their mental state is in question, insinuating that they should be sent to the ER (Emergency Room)

Bill: did you see John down a whole bottle of vodka at the Stacy’s party?

Bryce: Yeah, he also had a job interview the next morning, he showed up half wasted

Bill: That’s ER.
