Usually used by black people. It means that they aren't just a thug they are a REAL thug. As in they believe they are hard core.

The inside joke created by Kherington and the rest of the dancers on So You Think You Can Dance. Meaning for real, the IV being the roman numeral for four. The sign being, reaching your left arm up and placing the pinky, ring finger, middle finger, and pointer finger on the side of your shoulder that points the same way as your face and your thumb on the back of your shoulder.
