This man is a PANDA: During South Africa under Apartheid, segregation and oppression of non-white races, and domination of them by the white race resulted in financially, as well as politically, advantaged white families—now, decades later, these same men gawk at the idea of a equal society where he is now disadvantaged, and the very race he said could not do it is now advantaged—so advantaged in fact that the doctor, despite the patient's racist remarks and microaggressions, will treat the patient as an equal purely on principalism.
