Basically, The person will hack someone's account in a certain game and they get banned by the person who is hacking them.
Example:

Person 1: OMFG JENNY IS HACKING ME
Person 2: HURRY AND MAKE A NEW ACCOUNT
Person 1: OH NO I GOT BANNED
Person 2: NOOOOOOOOOO

Short for "hackneyed." Something that used to be cool, but has now become so mainstream/overrated that it's no longer cool.
My favorite TV show used to be The Office, but now it's hack, so I can't really say that anymore.

Smear Hack

Smear Hacking, also known as Smear Stalking, is a cyber hate crime intended to destroy a persons’ life. The hacker gains access to the victims’ smart phone or similar device and tracks them everywhere they go using the devices' GPS system. They then contact friends, neighbors, coworkers, etc. and try to ruin the victim with a campaign of insults and accusations. When Hate Groups use this tactic it is often called Gang Stalking.

Hack and Flack

A Hack and Flack is an informal gathering of journalists (often called 'hacks') and public relations people (often called 'flacks'). This is usually off the record and involves alcohol. The purpose is to build and improve relationships between two groups who often work together but sometimes for competing purposes.
I heard Joe is coming to the hack and flack tonight, it would be good to hear him not spitting out rote talking points for a change.
