It happens in environments where there are very high status people but with no hierarchical context or idea of who the others are.
The term was coined for the World Economic Forum (WEF), held in Davos. Because high status attendees are not allowed to bring an entourage, no one knows who is higher in status and they habitually walk into each other because everyone refuses to give way.

Often due to the creator or someone involved in said media being problematic in some shape or form (Racist, homophobic, ableist, etc). Enjoyers of a fandom usually don’t follow the same beliefs as the problematic person, and if they do, then that is a different issue.
Often the harassment comes from members of a far worse community. Attackers in the Danganronpa Effect often refuse to listen to reason due to their hatred for the series.
