It requires rapidly hitting the pen 5 times one after the other (start with one 5 sec hit then 4 sec hits the rest of the way) then hold the combination in for eight seconds. "Egg" describes the sensation that the smoker experiences in their throat. It will feel as though they have an egg in their throat.
