One can add the "2018" suffix to further underscore the expression's meaning.

In ancient times, men who had a microscopic penis (typically genetic) often had it removed or cauterized. When an adolescent male was found to have such a condition it was referred to as a Bonsimbiante - or absence of, at best, an average size Penis. The male penis was inverted inside of the patient and a fold was grafted to resemble a Vagina.
