The phrase refers to an anti-drug ad campaign that revolved around showing pictures of people in ruin with the phrase "Meth: not even once", general implication being that if you do meth even a single time it will suck you in and ruin your life.
The ad took off in the internet and was re-purposed, replacing "meth" with other things (X: not even once), generally meaning that the author of the image thinks that practicing X will lead to no good.

A term posers, like frat boys, use to brag up how many shots gets them the perfect amount of drunk while pregaming. When hanging out with other frat bags discussing drinking habits, this gives them the appearance of having amazing tolerance. When in actuality drinking this amount typically leaves them severely intoxicated and trying to hook up with some sorority freshman who now sleeps with any frat guy to get the attention she used to get in high school.
Dude we were over at (insert lame nickname here)'s and pregamed, yep did the even 9 so we are totally good to go man. Let's chug some boons farm, pop our collars, and jam some generic heavy metal.

A game similar to rock, papers, siccors. Usually played to determine which one of two people will have to perform a given act. One person calls out either odds or evens. They then proceed to count to three and each presents any number of fingers on one hand. The sum of the fingers from both participants is added up. If the sum is even the person who called evens wins, and vice versa.
