Maniac 

A word describing a person of particularly nasty or unpleasant personality, or one behaving in a hurtful manner towards others. Used in this context particularly in Israel, the term is often used to describe someone's military commander, boss, as well as any person or organization exerting rank or authority such as policemen or managers. Between friends, it may be used with a friendlier context.

"My boss asked me to work late today. he's such a maniac"
"That maniac cop pulled me over for driving 4 MPH over the limit"
"Those guys at the hardware store are maniacs. They dropped their prices right after I bought the lamp"
Between friends: "You forgot about our dinner plans, you maniac"

Taken from the game "Hitman," this exclamation was confined to the various Asian boards in the game and was said with a generically "Asian" accent. The full phrase was "weapon...weapon...watch out....maniac alarm!" The AI reserved such an exclamation for severe violations of civil decency on the part of the player/protagonist, such as walking into a Triad Den with a fully loaded ak47 and opening up for no good reason. The phrase can be applied, then, to any extremely antisocial behavior.

Everthing was going fine until you went totally maniac alarm on that dude.

Maniac Mansion

Another meaning is basically the NES game. This was a classic, one of the best NES games around, with lots of hilarious humour and mind-melting puzzles. The game revolves around the story of a weird mansion where a crazy scientist and his loopy, roleplay lovin' nurse wife captures a dumb blonde cheerleader girl, and you have to play as her boyfriend and two of his friends to rescue her from the mansion.

Good fun, jam packed full of crazy scientists, senile, kinky elderly nurse bitches, radioactive purple slime, punk-rock music, exploding hamsters, dead cousins and wax fruit munchin', Pepsi gulpin' "tentacles".
