Patootiest is derived from patootie, being short for sweet patootie, a term of endearment. So, for example, saying a child "is the patootiest" means that they are the "sweetest".

When the grandmother reviewed the comments made to a posting of a picture of her grandsons: "OMG what cutie patooties!" , she remarked, "They are the patootiest!"
