It's basically a hybrid of nut job and head case. The term would most likely be used in describing a young person who has not yet been proven insane but has a long history of disciplinary issues and unacceptable societal behaviors.

She is so sweet but that boy of hers is a real case job.

An extremely thin and sickly young woman who refuses to cook or clean for her seven bastard children and is typically seen holding a Virginia Slim in one hand and a flat-headed baby in the other.
