Someone who you fuck although every word out of their mouth is garbage. The metaphor is stuffing a gag in their mouth so you don't have to listen to their nonsense while you bang them. You could be fucking them because you're desperate. You might screw them because, despite being stupid, arrogant or full of themselves, they are extremely physically attractive or sexually skilful.
