Fuck up overdrive 

When a person is so deep into not following a given procedure you fear they will never be able to recover. Can also be used when someone is messing up a lot while doing various activities or being very forgetful.

Sometimes Bob goes into fuck up overdrive when he forgets to get his reports in on time.
