Doesn't really have to describe anything internal or external based on my experience. A way for somebody to describe themself as rougher/tougher than they actually are.
He never saw the roughness that the girl calling herself the diamond in the rough was describing in her title for herself.

Nobody really thought there was any roughness to the girl that called herself a diamond in the rough but herself, everybody else just thought she was a bully who was silly more often than not.

This phrase is generally only used by Cornish (Cornwall, UK) people- Something or someone is said to be “as rough as rats“. Alluring to having an unkept nature about it or them.
Person 1: Alright me fucker, hows you? ("Fucker" used as an affectionate reference to a friend, this is common in Cornwall)
Person 2: As rough as rats. I think I got the dreaded lurgy.
