The "third nipple" is non-existant because it's one too many body parts we need, so it's used to refer to when someone is over reacting to something or is about to say something that's exaggerative (<-is that a word?).

It's when men's nipples look like oversized pencil erasers. Permanently hard. If they're wearing a t-shirt you can see nipples from down the block. Sometimes they're natural and sometimes extreme nipple play and pinch twisting can make them that way after a long period of time.
