Taken from a lyric of Manfred Mann's "Blinded By The Light". A revved up douche is a douche of a person who is 'revved up', i.e., acting particularly douchey; someone being extra rude, loud, over the top, etc.
To be revved up like a douche is to be someone exhibiting behaviors like that of a douche that is revved up.

To race an engine, usually out of gear, to speeds at or past the rev limiter, making the engine emit a dramatic cacophony of high RPMs, exhaust noise and parts rattling from the vibration of the engine being put to it's limits.
To run the engine at such high speeds that the vibration risks the engine coming apart catastrophically.

When a musical note from a singer sounds like revving a car. It's mostly used in songs that lacks emotion. So the singer overcompensates by revving the note to get more emotion out of it.
Like in the song Soon by Leann Rimes, at the beginning where she sings "SooOOoooOOon" (Revving a note).
