This is originally a medical term taken from MDS Assessments. Question B1 is "Comatose" and an answer of 1 would indicate "yes".

Person 1 does or says something stupid

Person 2: oh god, you see that?
Person 3: yeah, B1=1.

Stupid, clueless. Used as a remark about someone who misses a joke (it goes over their head, like a 747, which is a type of plane)

Person 1: why'd the chicken cross the road?
Person 2: Why?
Person 1: to get to the other side
Person 2: hunh?
Person 1: oh god, 747...
