To pass out naked, most likely when drunk and male, on one's back, arms and legs spread out. Hence, starfish.

That one guy at the party that always has to get naked after getting completely smashed, and just so happens to pass out, still butt-naked, spread eagle on his back. Rockin' the starfish. Not a care in the world.
