It is a name used to compare someone or something to the icky black tar inside of a heavy smoker's lungs. Usually not meant to be used in a positive way.

Because of the chimney dust he had been around all morning, his face was coated in a thick ghaka.

Used to describe a person who has a vulgar sense of humor and/or has a slovenly appearance.

The veger hobo on the corner with the tattered clothing and the unshaven face meekly held up a cup for loose change.
