To completely humiliate yourself in any given situation in the worst possible way imaginable and being very susceptible to mistakes.

Cedric: Did you see how she stop in mid presentation of her mission statement to answer a random question and consequently transformed her face into a tomato that conveniently matched her hair. To make matters worst she failed because of it.

(In the workplace, especially in human services where several confidential documents might be combined into a single folder/chart).

The secretary had to folderize several client's charts.
