The reactor used in the Chernobyl nuclear disaster, had an issue where due to the control rod design it liked to blow up.

Dyatlov: do the test!
Akimov: no, not safe
Dyatlov: I dont care, do it
Akimov: ok
*emergency shutoff/scram button is pushed*
The RBMK now has a hole in it.
