A common call and response battle-cry for x-box live, stating that you will run down the middle of the map when the game starts on a suicide run. Common replies include, "Good Luck", "Your Dumb" and "STRAIGHT DOWN THE MIDDLE"

Number one rule of Minecraft. Doing so makes you fall in lava or often, falling into a cavern where you lose all of your stuff. Digging straight up does the same thing but with similar cases. Doing this also makes you get stuck in the hole you've dug in.

A woman who is Straight up and down like six o clock looks exactly like what the term implies---there are no boobs to break up the straight line appearance when you look at her from top to bottom. Most prominent when viewed from the side.
