How are you?

The lamest rhetorical question in the English language. Requires the answer of 'Fine' no matter how you feel or why you are going to have a conversation. An exercise for the lips and tongue.
I was dying of cancer and had to see my doctor. He asked me, "How are you?"

to ask your friend how he feels today, if there is anything you can do or say, how his voice is today and how much pressure he puts into a handshake. how his sleep is and what he eats.

Oft used as a do-not-bother-replying greeting, "how are you" represents the impersonalization of modern society stretched to an extent never witnessed before. It is however still okay to use it as long as you genuinely care.
EXAMPLE 1
Person A: Hi, how are you?
Person B: How are you?
(Conversation over)

Person B: Like you care.

How are you?

Not as simple a question as it seems at first glance. You usually say "I'm fine", no matter how you really feel – this is only for the closest people. Because explaining why you feel bad isn't very pleasant thing and few people won't take it seriously.
Usually when people ask how I'm doing(how are you?) , the real answer is I'm doing shitty, but I can't say I'm doing shitty, because I don't even have a good reason to be doing shitty. So if I say, "I'm doing shitty," then they say, "Why? What's wrong?" And I have to be like, "I don't know, all of it?" – Bojack Horseman

a phrase that a lot of people use to be kind. your response is supposed to be good, great, amazing and so on how about you, even if you don feel that way because the person that says this usually doesn't care about how you actually are.
