it is said when someone makes a bad situation worse by doing something else to upset you.
first the robbers snatched her chain, and then to add insult to injury , they molested her as well.

It means I'm already hurt so much that if you insult me it wouldn't even matter because I'm already so damaged(injured).
At this point you're adding insult to injury.

This is a distortion of an old expression: "adding insult to injury". The original meaning was that it's bad enough to be physically injured by someone, without receiving a verbal insult from the assailant as well.
"As if getting my parked car hit by a truck wasn't bad enough, the car being towed away by the parking police was just adding injury to insult."
