It refers to somebody that is VERY dirty and not clean OR somebody whom has not met your cleaniness standart .Opposite of Cleany.(there are 2 meanings of cleany please search both).
FOR EXAMPLE
You: Did you bathe before you ate dinner?
Me: No
You: I am a cleany and you are such a DIRTINI!
You: Did you bathe before you ate dinner?
Me: No
You: I am a cleany and you are such a DIRTINI!
