An event involving copious amounts of fail. Everything goes completely wrong and any attempts to fix it only result in making it worse thus leading to a cataclysmic disaster. Almost always caused by gross incompetence and/or a person with "mentally retarded" IQ classification as defined by the Wechsler Adult Intelligence Scales 1981 Classification.
