scrotcher 

A nefarious breed of bedbug which are almost impossible to eradicate. Scrotcher bites can be particularly painful, resulting in an ongoing itch around the crotch area which inspires incessant scratching.

He's scratching himself like that because his bed is infested with scrotchers.
