it basically means real talk, to be honest, or forreal

you: yu smarter.. and yu know wht it means so yu cn do it ..

me: iight..wat u mean im smarter

you: truth be told....jus makin u feel good so u will do it for me lol

OR

you: so why was everybody laughing at me at tha dance..thought u said i was looking good..

me: truth be told..i didnt feel like hurtin your feelings..plus you my ride home..l0l
