An intense form of the term, epic fail*. Refers to an action that typically results in fatal consequences.

*Epic Fail: A mistake of such monumental proportions that it requires its own term in order to sucessfully point out the unfathomable shortcomings of an individual or group.

"Her lethal fail was beyond epic proportions when she wore those hideous shoes in public."
