A cigarette. Used in the Twin Cities region, more specifically the town of Eden Prairie. Goes in conjunction with "busting a shmoo", which means to have a cigarette.

Gift From Grandma 

A handjob. Referred to as a "Gift From Grandma", because traditionally on holidays the gift from Grandma was the worst one given. Hence the analogy of receiving a gift you didn't really want.

Guy One: So how'd it go with Cheryl last night?
Guy Two: Eh, not so good. I just ended up with a gift from Grandma.
Guy One: Bummer, dude.
